DHS registers new students by appointment only. Call the Counseling Office at 770-232-3322 to schedule a registration appointment.

IMMUNIZATION FORM (Ga. Form 3231 - required by State Law)
Out-of-state immunization forms MUST be transferred to Georgia Form 3231. This can be done by a private doctor or by the Georgia Health Department.

HEARING/VISION/DENTAL FORM (Ga. Form 3300 - required by State Law)
Students entering from a private school or a school outside the State of Georgia are required to have a hearing/vision/dental exam recorded on Georgia Form 3300. This can be done at the Georgia Health Department.


TRANSCRIPTS AND WITHDRAWAL FORMS FROM PREVIOUS SCHOOL 
You must bring the most current transcript from the student's previous school indicating courses taken and credits accumulated. To insure the authenticity of the transcript, please have the sending school place the transcript in a sealed envelope with a signature or official school seal over the sealed enclosure. Withdrawal forms are also needed to place the student in the correct classes. If the student has been in Special Education or Self-Contained classes, we MUST have a copy of the latest IEP before registration can take place.


DISCIPLINE REPORT (or letter from school) (required by State Law) 
Under Georgia State Law O.C.G.A. 20-2.67, students cannot be admitted into any Georgia school without an indication of their discipline record. If the previous school has no discipline record form, please have a letter written o
n school letterhead stating
whether student did or did not have a discipline problem.
 


PROOF OF RESIDENCE
Parents must bring proof that they physically reside in the Duluth High School attendance area. The finalized purchase/lease agreement or two (2) current utility bills, phone bills, or bank statements in the parent's name with address are acceptable. PLEASE NOTE: If you are residing someplace other than your own home or apartment, you must produce a notarized statement from the person you are living with stating that you are residing with them and bring two (2) proofs of residence from that person. A driver's license or check is not acceptable.


GUARDIANSHIP
If you are not the natural parent (birth parent or adoptive parent), you must present legal guardianship documents from the Probate Court (770-822-8250) at the time of registration. Notarized letters are not acceptable.


SOCIAL SECURITY NUMBER
If the student plans to attend college with benefits from the HOPE Scholarship, the student's Social Security Number must be provided to the school. Please bring the Social Security card (or copy of it) to keep in student's file. However, you may decline to give the Social Security Number if you wish to forfeit Hope Grant money.


FOREIGN STUDENTS
If student has never attended school in the United States, he/she must first go through the International Newcomers Center before registering at Duluth High School. Call 678-377-8945 for an appointment.

WITHDRAWALS

  • Unless a student is 18 or older, a parent or guardian must be present to sign paperwork.
  • After paperwork is signed, withdrawal documents may be picked up 24 hours later.
  • Withdrawal documents will not be released until all textbooks, library books, and uniforms are returned and fines are paid.
